VAGINAL RECONSTRUCTION

LABIAL REDUCTION

The labia minora, which are the inner lips of the vulva, can be excessively long, either by heredity or as a result of childbirth.  This may merely be an esthetic problem you wish corrected. Or there may be discomfort riding a bicycle or horse, sometimes even just sitting down wrong.  The labia minora can also be swept into the vagina during sex, pulling uncomfortably or pulling in pubic hair which can be uncomfortable for both male and female.  The proper surgical  technique for reduction is not to simply trim them shorter.  Rather, they should be ballooned up with anesthetic and then more skin removed on the outside than the inside.  Then in suturing them, the scar line is not at the end of the labia, but rather on the outside edge where it cannot be rubbed during intercourse.

 

PARTIAL HOODECTOMY

The hood of the clitoris can cover it to an excessive degree, usually in association with elongated labia minora.  The hood can be reduced the same way the labia minora are.   There can also be adhesions from the hood to the clitoris.  These should be released.

 

REPAIR OF THE INTROITUS.

Introitus is the term for the vaginal opening.  Tears may occur during childbirth that do not heal correctly, or an episiotomy repair may come partially undone during healing.  This results in a loose vaginal opening.  This can be repaired by bringing the perineal muscles back together at the bottom of the opening.  It is very important that the high point of the vaginal opening be vaginal skin, not the skin further out.  If the skin further out is attached side to side at a higher level that the vaginal skin, it can get little tears in it during intercourse.  These tears can be quite painful, and the diagnosis is often not made correctly because they have healed by the time you get to your gynecologist.  Correcting this (transverse perineal band) is relatively simple if one understands the nature of the problem.

 

RECONSTRUCTION OF THE HYMEN

I don’t get many calls for this, but I have done it with very natural results. 

 

TUMMY TUCK

When skin is removed to pull the abdominal skin tight, there is also a pull on the vulva, pulling it tighter against the pubic bone.  Patients then realize that the vulva has been sagging along with the abdomen, and their reactions are all favorable.
